The undersigned (hereinafter, the "Players") hereby acknowledge to the Men's Senior Baseball League, Inc. and Men's Adult Baseball League, Inc. (individually and/or collectively, "MSBL") recognition of the fact that medical insurance is not inherent with national or local league membership.
WHEREAS, Players shall participate in the MSBL as a member on a team called _______________, which is a "Team" in the Tucson Men's Senior Baseball League the "Local League".
WHEREAS, Medical insurance may be purchased by any or all affiliates of MSBL through the national organization to all Local Leagues, through league director correspondence.
WHEREAS, Team may elect, on a Team basis, to purchase medical insurance for their Team independently or as made available through MSBL. Such coverage would include coverage for any injury sustained from participating in a MSBL sanctioned game or practice as provided under the terms and limitations of the insurance Policy. Such insurance would be purchased directly through Gagliardi Bros. Insurance.
We, the Players, have read this release and understand that it is an absolute acknowledgement and recognition and we freely and voluntarily accept its terms.
